MDI Questionnaires

 

The Middle Years Development Instrument (MDI) is a self-report questionnaire that asks children in Grades 4 through 8 about their thoughts, feelings and experiences in school and in the community. 

The MDI questionnaire is slightly different for each age group. For example, it has 77 questions for Grades 4 and 5 and 101 questions for Grades 6, 7, and 8, with additional questions focusing on Self-Awareness, Perseverance, Assertiveness, and Citizenship and Social Responsibility. 

We encourage parents and caregivers to view the questionnaires. Some families use questions from the MDI to help guide conversations about the child’s feelings and experiences in school and in the community. Some of the questions on the MDI might be on topics that are new to children and therefore might cause them to ask their parents, caregivers or teachers questions about these topics. MDI Canada

MDI Canada sites, including school boards in Northwest Territories, Yukon, Ontario, and Nova Scotia, may use slightly different MDI questionnaires. Please contact Maddison Spenrath to learn more about MDI Canada.
